Application Process

For Online:


  1. Download the application form.
  2. Excel file
  3. PDF file
  4. Fill out all fields except for Payment Information.
  5. Pay the registration fee (P500) through the payment modes below.
  6. Input details in this Google Form and upload the following files:
  7. Application form (With the file name format: Last Name, First Name_Strand)
  8. Proof of payment (Receipt or screenshot)

For Onsite:


  1. Visit our school at Brgy. Cadulawan, Minglanilla, Cebu.
  2. Fill out the application form provided.
  3. Pay the registration fee (P500) through cash or through the payment modes below.
  4. Present the proof of payment.
  5. Prepare the necessary admission requirements for submission on or before the start of classes.

Senior High School (Grade 11) Requirements:


The following requirements must be submitted to the Registrar’s Office before admission to any academic program:


  1. Original Form 138 / SF9-JHS (Learner’s Progress Report Card)
  2. Original Form 137 / SF10-JHS (Learner’s Permanent Academic Record)
  3. PSA issued Birth Certificate
